A school enterer is any child between the ages of 2 months and 21 years entering or being admitted to a Delaware school district for the first time, including but not limited to: foreign exchange students, immigrants, students being admitted from other states and territories, and children entering from non-public schools. REQUIREMENTS FOR SCHOOL ENTERERS: IMMUNIZATIONS: Five doses of DPT, DTaP, or DT vaccine Four doses of OPV or IPV Three doses of Hepatitis B Two doses of MMR Two doses of Varicella or proof of disease. Blood lead testing with results for all pre-k and kindergarten students. PHYSICAL EXAMINATION: Physcial exam completed by a licensed medical physician, nurse practitioner, or physician's assisstant. The physical exam must be current within the past 24 months. Starting 2012/2013 school year, all 9th grade students must have an updated physicial examination on file in the nurse office. MANTOUX TUBERCULIN SKIN TEST OR RISK ASSESSMENT: Proof of Mantoux tuberculin skin testing results or TB risk assessment within the past 12 months. Known positive reactors need verification from their physician or Division of Public Health for results and completion of preventative therapy for TB infection or chemotherapy for TB disease.
